Transportation Agreement
Anishinaabe Endaad staff will help you build the resources and skills around transportation to aid in meeting your goals. This may include connection to transportation resources through health insurance, church/community volunteers, budgeting, or making connections within your natural support system. In certain cases, Anishinaabe Endaad staff may assist with providing transportation. To promote safe transportation, the following transportation agreement must be signed by the participant.
By signing this document, you agree to the following.
You understand that Anishinaabe Endaad staff...
- May not always be available to provide transportation and are not required to transport you or your belongings.
- Are not responsible for lost, stolen, or items that are damaged when providing transportation.
- Are not responsible for lost, stolen, or items that are damaged when providing transportation.
- Have the right to refuse transportation or may ask you to leave the vehicle if you are under the influence of substances, unable to stay calm, or behaving in a way that may put Anishinaabe Endaad staff in danger.
- Can only transport you to approved locations and cannot transport friends and family.
- Has the right to restrict transportation temporarily or permanently if you are in or have been in violation of the transportation agreement.
You understand that you will...
- Not be under the influence or carry any alcohol, drugs, paraphernalia, or weapons in a staff's vehicle.
- Be respectful to Anishinaabe Endaad staff, their vehicles, and their property.
- Remain law-abiding, including wearing a seat belt.
- Respect the right to privacy of others with whom you may be riding with to activities coordinated by Anishinaabe Endaad.
