Welcome to Danielle Fields Salon. I can’t wait to meet you! All new guests begin our working relationship here with this digital consultation form. This allows me to get to know you and your hair goals so that I can ensure your initial appointment is booked properly. Please complete the form in full and I’ll be in touch within 24-48 hours of your request during my work week which is Monday-Thursday. By completing this form, I understand that Danielle Fields Salon has a strict 48 hour cancellation policy and if I do cancel my appointment with less than 48 hours notice, I may be charged 50% of the service cost. For day of cancellations and no shows I may be charged 100% of the service cost.

Your appointment in the salon will begin with a full consultation to refine the plan for service. If you have more pictures this is the time we can talk this over.
