These guidance notes are an important part of the contract and should be read and understood as the contract is completed. Details in the contract are confidential, except in the case of safeguarding concerns. Please note that Ofsted, HMRC, Tax Credits and other Government agencies have a legal right to examine all documents including contracts and may request to view contracts or record parent details.
Please read these notes as you work through the contract.
1. Provider Details - This section will be completed by your childcare provider. Please note your providers' contact telephone numbers and add these to your contacts on your mobile phone. The additional telephone numbers listed are only to be used by parents in case of emergency. Note: It is a requirement of the Early Years Register that your provider holds current, up-to-date Public Liability Insurance. You should ask to see their Public Liability Insurance Certificate as well as their Ofsted Registration Certificate if this is not shared with you.
2. Child Details - Please include your childs' full name and date of birth. Add their address if it is different from your address. Include the name your child prefers to be called. Include any specific details such as educational needs, disabilities, medical conditions, allergies and food intolerances.
3. Family Details - Please include the name(s), address(s) and telephone numbers of all people with parental responsibilty for your child. In cases of shared custody, include details of residence or contact arrangements as well the address of each parent and the address at which the child is resident. The contract must include details of all adults who have parental responsibily for the child. This information is required by law. 4. Start Date - Include the date your child will start at the setting.
5. Days/Hours - Please fill in the arrival and collection times for the days your child will attend the setting. The hours stated on the contract are available for the sole use of the child named in the contract and will be charged in advance unless notice (in writing) of changes is given by the parents. It is important to consider travel times to and from your place of work when setting arrival and collection times.
Note: Fees charged in advance will be refunded if the provider is unable to provide a service, less any reasonable advance expenses, to comply with consumer law.
6. Additional information - Please state if your child will attend full time (term time and holidays). Note that the contract is legally enforceable and fees must be paid for all booked hours. Please read the Holiday Policy for more information. Please state the meals you wish us to provide. Unless you are receiving funding, all meals are included in your fees. Please provide details of your childs' school or preschool. This is where you can state the hours your child will be in school/preschool. Please supply the term dates for your childs' school/prescool at the beginning of each academic year.
7. Fees - Full time is more than 6 hours per day and more than 12 hours per week. Part time is 6 hours or less per day and 12 hours or less per week. As funding only covers actual childcare, sundries is a daily fee charged to parents to cover food, wet wipes, outing and other consumables. Should a parent not wish to pay this fee, they can provide these themselves. This can be discussed prior to signing contracts.
Invoices are sent out the week of the 25th of the month and payable by the end of the first week of the month, in advance. (Eg: Invoices for August will be sent out around the 25th July and will be payable at the beginning of August). Any additional hours your child attends, outside of their contracted hours, (either if staying late or additional days) will be included as extra days on the following months invoice. If you wish to dispute an invoice, this must be done within 7 days of receiving the invoice in question. After this time, no changes will be made to any invoices.
Please ensure that you have read the policy on late payments.
The setting closes for 2 weeks over the Christmas period (exact dates will be provided) and on public holidays. No fees are payable for these days.
8. To be completed by the parent / guardian - Please tick all statements that you agree to . Permission forms will be sent to you with your contract. Policies are on our website and a password will be provided so that you can view these. When policies are reviewed, if any changes are made, you will be notified via email. Once you have completed and signed your sections, the contract will be signed by the childminder.
9. To be completed by the childminder - This section will be completed and signed by the childminder once the parent / carer has completed filling in their sections of the contract. The contract will be reviewed every year and changes (if any) will be noted and the review signed. Any changes will be noted on the additonal information page. The existing contract stays in force until the review is done.
