Protecting your personal and billing information is very important to us. Once you submit this form, all your personal and billing information will be encrypted and sent directly to the Office of Faith Formation. We use Stripe to process all credit/debit card transactions; they too, place the security of your information in the highest regard, ensuring that all data is sent over a secure HTTPS connection.

Sacraments

All sacraments are meant to be prepared for and received within the life of the parish. If you are registering your children for our regular Faith Formation Sessions, then they will be prepared throughout the year; however, if your children are enrolled in an elementary Catholic school, then they will need to take part in sacramental preparation in the parish.

A schedule will be forthcoming for sacramental preparation. 

Sacramental Preparation

Under each child you are registering, you will be asked if they will be preparing to receive a sacrament this year. In general, one who is in 2nd Grade will be preparing for First Reconciliation and one in 3rd Grade will be preparing for both First Eucharist and Confirmation.

If your youth is in the 4th Grade or higher and has not yet received the Sacrament of Confirmation, but has receive their First Eucharist, please select "Confirmation Only".

If your child is preparing for one sacrament this year, the fee is $50. If your child is preparing for Eucharist and Confirmation this year, the total sacrament fee is $75.

Medical Forms

If your child needs an epi-pen, inhaler or other medical equipment on hand during a session or even, please contact the Faith Formation Office directly and note it in the space provided above.

Photographs are sometimes taken during Faith Formation sessions/events. If you do not want images taken and used as described, please send a written notice to that effect to the Director of Faith Formation at the address on the heading of this form.

To double check to make sure the correct final balance has been calculated use the following values and formula:

1 Sacrament: $50  |  2 Sacraments: $75

Family Tuition Fee: $150

Number of Sacraments + Family Tuition = TOTAL

TOTAL * .20 = Deposit Due Today

Example: 50 + 150 = 200

  200 * .20 = 40

You have chosen to break the total cost into payments. By using the monthly payment option, you will be required to make a 20% deposit of the total fee now, by using the payment information below.

The remaing balance will be broken up into five equal monthly payments, and the debit/credit card that you provide below will be charged on the first Monday of every month, starting in November and ending in March.

For example, if your registration fees total $200, your required deposit will be $40. Your remaining balance of $160 will be broken into five equal payments of $32, which will begin on the first Monday in November. 

Note: The Total Tuition should not exceed $300 per family.
