I understand there may be risks in touring a manufacturing plant and I may be exposed to potential hazards, including but not limited to: moving machinery, chemicals, and industrial equipment. I acknowledge that the manufacturing company will take reasonable measures to ensure the safety of its visitors and that it will provide all required safety equipment. I agree to comply with all safety rules and regulations and will use the safety equipment provided. I understand that all tour participants must come to the tour wearing closed toe shoes. I understand that neither Valley Industrial Association (VIA), Greater Chicago Advanced Manufacturing Partnership (GCAMP), nor the manufacturing company will be held responsible for any injuries or damages that may occur during the tour. I waive all claims against VIA, GCAMP, and the manufacturing company its officers, employees, agents, and representatives for any and all injuries, damages, or losses that I may sustain as a result of participating in the tour.
Photo Release

I grant GCAMP and its parent organization, VIA, permission to take photographs of me during the tour. I understand photos may be used for reasons including and not limited to: publicity, illustration, advertising, and web content. I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use of any photos taken.
Transportation & Safety Requirements

Participants are responsible for securing their own rides to and from tour locations. Participants should arrive 15 prior to the start of the tour to check-in. Rides home should arrive five minutes before the end of the tour to pick up participants. Onsite parking is available at each location. All tour participants must come to the tour wearing closed-toe shoes.
