Summer Buddy Membership Request Form

In consideration of the above factors and in consideration of the privilege of participating in the Fitness and Recreation area of the Beaman Center activities, the undersigned participant acknowledges the existence of risks in connection with the facilities and/or equipment.  In addition, participation releases Belmont University, its officers, trustees, employees, students, and agents from any liability for claims arising out of the use of exercise equipment, participation in supervised and unsupervised activities in the gym, and in other individual or group exercise activities, and accidents occurring in the locker rooms and shower facilities of the center.

The participant recognizes that possible injuries which may arise out of participation in the center’s activities include, but are not limited to, potentially fatal injuries such as heart attacks, stroke, and heat stress, as well as other injuries such as back injuries, broken bones, torn ligaments, etc. The participant releases Belmont University from liability for such injuries regardless of whether
his or her damage or loss is caused by any negligent act or failure to act of the university or otherwise.

Belmont University reserves the right to restrict use of the Fitness and Recreation area of the Beaman Center in instances where the member has a history of health problems that would render the member unqualified to participate in certain aspects of the center’s programs and activities. All decisions concerning restricted use will be made by the University after consultation with the member’s physician.

The participant further acknowledges the existence of any need for certain rules concerning the use of equipment, facilities, and other procedures related to the Fitness and Recreation area of the Beaman Center. Each participant agrees to abide by those rules and to make every individual effort to assure that the equipment and facilities are kept in safe and useable condition.

Having read the preceding, the participant acknowledges an understanding of those risks set forward and knowingly agrees full responsibility for any exposure to such risks. I understand that this membership is non-refundable and non-transferable.

Summer 2024 Buddy Membership Information

• The 2024 Summer Buddy Membership Term is May 6th-August 9th. Regardless of the membership inception date, all memberships expire on August 9th.
• Buddy members will not be permitted to sponsor guests to use the Fitness and Recreation areas of the Beaman Center. Buddy members are not permitted to sponsor other individuals for a Buddy Membership.
• Buddy members must 16 years of age or older.
• Buddy members agree to follow all policies and procedures of the Beaman Center. Non-compliance may result in termination of membership.
• Buddy membership fees are non-refundable.
• I acknowledge that the Fitness Center will be closed for several holidays throughout the summer. They are as follows: Memorial Day, Independence Day and Maintenance Days/Week which is TBD but will be posted in the facility and on FitRec social media accounts.
• The Department of Fitness and Recreation reserves the right to alter the hours of operation of the Fitness and Recreation facilities for special events at any time during the Summer.
• The Rec Gym will be used periodically throughout the summer for Sport Camps. 

I hereby acknowledge that I am responsible for any individual, and their actions, I sponsor as a Buddy Member. If the individual that I sponsor for this Buddy Membership fails to adhere to any of the policies and procedures of the Beaman Center and Belmont University, my current membership and/or membership status is subject to penalty. I also acknowledge that I am permitted to sponsor 2 Buddy Members in a given Summer.
