Goodwill Program Transportation Agreement
Welcome to the Goodwill program! As part of our services, we strive to provide reliable transportation to support you in achieving your goals. Please read and sign below to acknowledge your understanding and agreement to the transportation guidelines. These guidelines apply to all programs; however, transportation services may not be available for every program.
Transportation Guidelines:
Appointment Transportation
- During program hours (8:00 AM - 2:30 PM), Goodwill transportation may be available to drop you off and pick you up from approved appointments.
- All appointments must be arranged with your Case Manager at least 24 hours in advance.
Service Area
- Goodwill transportation operates within Wooster city limits.
- Transportation requests outside of Wooster city limits must be approved by the Mission Services Director.
End-of-Day Appointments
- The latest appointment time eligible for Goodwill transportation return service is 1:00 PM.
- If your appointment extends past 1:00 PM, you will need to arrange your own transportation home.
Punctuality
- You must report for transportation at your scheduled departure time.
- If you are late reporting, transportation to your appointment will not be provided that day.
Pick-Up After Appointments
- After being dropped off, you must wait at least one hour from your scheduled appointment time for transportation to return and pick you up.
- If your appointment will last longer than one hour, you must call the number provided to notify transportation of the delay.
Calling Off
- Call 330-287-5050 to report your cancellation. This is a designated call off line. If no one answers, leave a detailed message explaining you reason for calling.
- If calling off for the day call off before 7:30. Someone checks this line regularly and will receive your message promptly.
Acknowledgment and Agreement: By signing below, you acknowledge that you have read, understood, and agree to abide by the above transportation guidelines. Failure to follow these guidelines may result in suspension of transportation services.
