I understand that if selected as Senior Team Leader, I will comply with the following:
 
I will not, under any circumstance, bring drugs, alcohol, or other related items to camp.
 
I understand that to eliminate distractions, mobile phones are not permitted for leaders or campers. With this in mind, I will either not bring a cell phone to camp OR agree to have it stored and returned to me at the end of camp.
 
I will value and demonstrate appropriate behavior at camp, understanding that the words I say, my attitude, and my conduct have an effect on the experience that I, my campers, and my peers have at camp.

I will consent to a background check to be performed by TYM and its affiliate partners.

I understand that this is an application for and not a commitment or promise of volunteer opportunity. I certify that I have and will provide information throughout the selection process, including on this application for a volunteer position and in interviews with Today's Youth Matter, that is true, correct, and complete to the best of my knowledge. I certify that I have and will answer all questions to the best of my ability and that I have not and will not withhold any information that would unfavorably affect my application for a volunteer position. I understand that the information contained on my application will be verified by Today's Youth Matter. I understand that misrepresentations or omissions may be the cause of my immediate rejection as an applicant for a volunteer position with Today's Youth Matter or my termination as a volunteer.

Media Consent to Release

Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to Today's Youth Mater (TYM), its affiliates, and agents to use my image and likeness and/or any interview statements from me in its publications, advertising or other media activities (including the Internet).  This consent includes, but is not limited to:

            (a) Permission to interview, film, photograph, tape, or otherwise make a video reproduction of me and/or record my voice;

            (b) Permission to use my name; and

            (c) Permission to use quotes from the interview(s) (or excerpts of such quotes), the film, photograph(s), tape(s) or reproduction(s) of me, and/or recording of my voice, in part or in whole, in its publications, in newspapers, magazines and other print media, on television, radio and electronic media (including the Internet), in theatrical media and/or in mailings for educational and awareness.

This consent is given in perpetuity, and does not require prior approval by me.
