Completion and submission of form [Please read]
This form is for members who wish to exhibit, and members who would be willing to help, but not exhibit.
Please offer as much help as you can. PACS relies on members giving up their time to help events to be held. The more offers we receive then the fewer duties individual members will be asked to carry out.
Please provide as much relevant information as possible, as this is really helpful. Where a form field is marked with a red asterisk (*), a response must be provided.
You need to allow enough time to complete your form in one sitting (you cannot save it and come back later), so please have your diary to hand before starting. You can close the form at any time but none of the data entered will be saved.
Once completed, please submit the using the "Submit" button at the end. Upon submission, you will receive a message on your screen saying "Thank You. Your submission has been received". You will not be able to amend your form after submission, so please contact a member of the exhibition team urgently if your circumstances change. email exhibition@petersfieldartsandcrafts.org.uk
You may wish to make a note or screenshot of your entry before submission. However, if you wish to receive a reply e-mail with a copy of your completed form, please provide your email address below (for the sole purpose of receiving this reply). The reply will be sent from a "no-reply" email address, so if not received please check your junk/spam folder.
