Step 1: FILL IN ONLINE APPLICATION
Fields marked with an * MUST be filled in for the application to submit.
Step 2: MAKE PAYMENT
Payments MUST be submitted by the cut-off date, June 16th, to make sure you are guaranteed a pinnie for your daughter. First 50 will get a custom NIKE Hoover Lacrosse Pinnie. You may venmo @NorthCanton-HooverLadyLax and include your daughters name and grade. You may also mail a check to Hoover Lady Lacrosse, PO Box 2297, North Canton, OH 44720. Make sure if you mail a check to include players name and grade. **You will receive an email when your payment is received in full.
