Emergency Supplies
Evolve PLAY I TRAIN Emergency Preparedness Program requires that every child is provided at school, a comfort kit, designed to provide some short-term supplies and, more importantly, reassurance and comfort for your child in the event of an emergency or of an earthquake.
Evolve PLAY I TRAIN comfort kits contain supplies recommended by your Health Authority including such items as drinks and snacks. You will be charged a fee for the cost of the kit.
Out of Area Contact
Evolve PLAY I TRAIN is involved in an ongoing Emergency Preparedness Program. To ensure the safety of the children and staff, precautions - such as fire and earthquake drills are practiced regularly. An important part of this program involves issuing and maintaining personal photo ID tags. They would provide vital medical information in addition to assisting with a safe and speedy communication and reunion with family. This tag is for each individual child and includes information such as authorized persons to release the child to, name of the child, and other important details.
This form should be kept current, and any changes should be applied immediately, and the school office notified.
Considerations:
1. Alternative Person - In the event of a major emergency, your child would be released only to you (as a Parent or Guardian) or to one of the two individuals listed as Alternates in the Enrollment Form.
2. If possible, siblings should have the same Alternate Persons named to keep the family group together.
3. Out-of-Area Contact - In an emergency such as an earthquake, it is likely there would be considerable disruption to the telephone service. Families should plan to have an Out-of-Area Contact who could act as a central point for relaying of messages. It is recommended that your Contact Person be a minimum of 160km from your school's location.
4. Both the Alternate Persons and the Out-of-Area Contact need to know that they have been named and be aware of their role in the event of an emergency.
