2023-2024 Island Flyers

My child, listed below in the applicant section of this form, has my permission to tryout to be an Island Flyer at Rise Academy of Arts. I understand that he/she must abide by the rules and regulations set forth by the staff and the owners of Rise Academy of Arts, and be present for all specified tryout days. I understand that my child must be a member of or obtain a membership at Rise and we must pay the tryout fee prior to tryouts. I understand that my daughter/son will be evaluated by qualified judges, and we agree to abide by the decision of the judges. I understand all costs involved as stated on the tryout flier. I understand by the very nature of the activity, gymnastics carry risks of physical injury. No matter how careful the participant and coach are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated. The risk of injury includes injuries such as muscle pulls, dislocation, and broken bones. The risk also includes catastrophic injuries such as permanent paralysis or even death from landing or falls on the back, neck, or head. I understand that sanitizing procedures and protocols will be in place but contracting Covid is a risk of group sports and activities. I understand these risks and will not hold Rise Academy of Arts or any of its personnel responsible in the case of accident or injury at any time.

I am interested in being an Island Flyer at Rise Academy of Arts. I understand the risks stated above. If elected, I promise to abide by the rules and regulations set forth by the Island Flyers Contract as well as the coaches and the owners of Rise Academy of Arts.
