Risk Assessment Request Form

The contractor is responsible for ensuring that any required actions or precautionary measures that are noted on the risk assessment form are in place prior to the start date of the contract.

Contractor audits must be completed and submitted on line as soon as the property goes live and again any required actions will be the responsibility of the contractor to follow up and ensure items are in place prior to the start date of the contract.
