The Sower Church Youth Soccer Camp returns for another summer. This is our seventh year of our free youth soccer camp, allowing us to serve the Lincoln community and families by offering free soccer lessons for kids ages 3-13. The camp will be held the first three Saturdays in June starting June 3rd at 10 am at Speedway Sports Complex (315 Speedway Circle, Field 1C).

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Sower Church during the selected camp. In exchange for the acceptance of said child’s participation, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Sower Church and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Sower Church including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

The participating child(ren) are recommended to wear athletic clothes and tennis shoes. Protective gears, such as shin guards, soccer socks, soccer shoes are strongly recommended.

The participants’ parent(s) are recommended to be present before, during, and after each activity. It is within the discretion of the Organizer and its volunteers to ask any child to sit out and to not participate for safety reasons.
Privacy Policy

Any information received in this form and during the activity, such as names, photos, and videos will be used for the sole purpose of developing and advertising this activity. Such information will not be transmitted to a third party without the consent of the parent Or legal guardian of the participant.
Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
