Health or Medical Issues
I understand that grooming can be stressful to some pets and I will inform the groomer if my pet has any heart conditions or any stress related issues prior to grooming. I understand that if the groomer feels that continuing the appointment would be harmful to my pet or their safety, the groomer will contact me and/or cancel my appointment.
Emergencies
In the very unlikely event of an emergency, I authorize this groomer to recommend professional veterinary attention for my pet and I agree to immediately provide such care (at my expense). I understand that all attempts will be made to contact me in the event of an emergency or injury.
Refusal of Services & Reactive Clients
The groomer reserves the right to refuse any service at any time before, during, or after grooming if my pet is too agitated or becomes unsafe to groom. However, all efforts will be made to keep my pet comfortable, including the potential use of a well-fitted muzzle, my presence during the groom, or any additional resources that may be necessary. In the unlikely event that the appointment is cut short, I understand that I may still be charged a grooming fee based on what has been done up to that point.
Parasites
If my pet has fleas or ticks, the groomer may recommend treatment elsewhere and reschedule my appointment.
Vaccines
Vaccination is strongly encouraged for all pets, and Rabies immunization is required prior to appointments.
Coat Condition
I understand that the groomer may have to adjust the groom depending on the condition of my pet (excessive matting, skin condition, other unexpected barriers). I will be informed of any necessary changes as they are needed. I also understand that if my pet is severely matted, that there is an increased risk for clipper burn or small cuts to occur. I understand that all attempts will be made to prevent this, but in extreme cases, it may be unavoidable.
Pricing
Pricing operates on a sliding scale based on the needs and conditions of my pet. Specifics may vary depending on financial status, size of my pet, fur length/texture, demeanour of my pet, and time needed to groom - among other factors. All prices are open to discussion, please contact the groomer.
Equipment
If any additional equipment is required for my pet's appointment, a small (agreed-upon) fee will be added to my appointment cost. If desired, this additional fee can be put towards the purchase of the equipment for personal use at the end of my appointment.
Cancellation Policy
I understand that if I need to change my appointment time or cancel it, then I must give at least a 24 hour notice so that the appointment time can be made available.
Photography
I give the groomer permission to photograph my pet for use in customer files and their personal records. Any use in advertisement or websites/pages can be discussed separately.
I have reviewed this Consent Form for accuracy, and I affirm that I am the rightful legal owner of the pet(s) for which services are being rendered. I authorize this contract to be valid approval for future grooming services without additional signed contracts or written authorization. I understand that signing below constitutes a legal signature confirming that I acknowledge and agree to the above terms.
