The Code N Bots Development teams meet twice a week for 72 classes.

On Tuesdays' the team meets: August 22,29 September 5,12,19,26 October 3,10,17,24,31 November 2,7,14,21,28 December 5, 12 January 9,16,23,30 Feb 6,13,20,27 March 5,12,19,26 April 9,16,23,30 May 7,14,21. One of these dates is extra and will be a floating absence day for the instructor in case of illness. 

On Thursdays' the team meets: August 24,31 September 7,14,21,28 October 5,12,26 November 2,9,16,30, December 7,14, January 4,11,18,25, February 1,8,15,22,29 March 7,14,21,28 April 11,18,25, May 2,9,16,23,30.

Possible additional days are Friday April 12 and May 10th, which will be added in case of instructor illness. 

Payment Schedule

First and last months payment reserve your spot in the class, check the box to save your payment method for reoccuring payments.  You are authorizing Code N Bots to bill and process a monthly fee of $149 for development team meetings starting after 3pm and $134.10  for classes before 3pm on the 24th of each month from September 24th through March 24, 2024.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Code N Bots during the selected camp. In exchange for the acceptance of said child’s candidacy by  Code N Bots, I assume all risks and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Code N Bots and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Code N Bots including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. 

Medical Release and Authorization

As Parent and/or Guardian of the named student, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination for the named student. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to Code N Bots and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
