We, Parent/Guardian (full name)* (parent/guardian initial, if under 18) hereby give permission for our son/daughter; to travel from Calgary, AB to Seattle, Washington from June 9-11,2023.

Our son/daughter is attending the Ted Houk Regatta with the Calgary Canoe Club. Calgary Canoe Club coaches will supervise athletes.

I give my permission for medical assistance to be administered should they become ill or involved in an accident initial* (parent/guardian initial, over 18+).
Should medical treatment be needed while out of country the parents will be contacted ASAP. CCC has collected information when you join the program but did not collect medical insurance documents. Attach image of insurance card or policy info below.*
