Permission Form and Medical
We, Parent/Guardian (full name)* (parent/guardian initial, if under 18), hereby permit our son/daughter to travel from Calgary, AB to Seattle, Washington from June 11-13, 2025.

Our son/daughter is attending the Ted Houk Regatta with the Calgary Canoe Club. Calgary Canoe Club coaches will supervise athletes.

Calgary Canoe Club supervising coaches for the event are Mike Robinson and Robin Kendall. Coach Kristiane Free will attend the event as a competitor and may fulfill some coaching roles.

I give my permission for medical assistance to be administered should they become ill or involved in an accident initial* (parent/guardian initial, over 18+).
Should medical treatment be needed while out of country the parents will be contacted ASAP. CCC has collected information when you join the program but did not collect medical insurance documents. Attach image of insurance card or policy info below.*
