This form is to be completed for the proposed transfer of the apprenticeship of an Apprentice Jockey. An application will not be considered unless it has been completed in full and has been signed by the current approved employer of the Apprentice Jockey, the proposed new employer of the Apprentice Jockey and the Apprentice Jockey, and all of the required attachments have been submitted.

By signing and submitting this form: 

1. The current approved employer of the Apprentice Jockey agrees to the transfer to the proposed new employer, as from the date of transfer, of all of their rights and obligations under the Memorandum of Understanding – Approved Employer previously registered with NZTR between the current approved employer and NZTR and under the Agreement of
   Apprenticeship (being the individual employment agreement entered into between the current approved employer and the Apprentice Jockey/Probationer).
2. The new employer agrees to accept the transfer of such Memorandum of Understanding – Approved Employer and Agreement of Apprenticeship, and to fully and faithfully perform all the obligations of the current approved employer under the Memorandum of Understanding – Approved Employer and the Agreement of Apprenticeship, in each case as from the date of transfer.
3. The Apprentice Jockey/Probationer agrees that, as from the date of transfer, they will accept the new employer as their employer under the
   Agreement of Apprenticeship and the Memorandum of Understanding – Approved Employer for the remainder of the term of each Agreement in all respects as if the new employer was the current approved employer and that the current approved employer will be released from all obligations to be performed by the employer under the Memorandum of Understanding – Approved Employer and the Agreement of Apprenticeship after that date.
4. The Apprentice Jockey/Probationer acknowledges and agrees that, in accordance with the terms of the Memorandum of Understanding – Approved Employer and the Agreement of Apprenticeship, 50% of any race day earnings will be paid to their employer and the remaining 50% will be held in Trust by NZTR for the duration of the Agreement of Apprenticeship.

Although all the signatures below are necessary, NZTR acknowledges the potential logistical difficulties in obtaining them. The signature of the applicantis required, while NZTR will facilitate obtaining the remaining signatures.

You will be notified of the outcome of this application by the NZTR Licensing Panel by email. If the application is approved, NZTR will send a notification advising that it has been approved and outlining any terms and conditions imposed as part of its approval.
