There are four sections to the application: Applicant's Personal Particulars, Applicant's Profile, Interest in Fellowship and Declaration.

As part of the submission process, you will be required to provide your most recent CV along with relevant assets such as websites and supporting documents of 3 arts-based community projects you have been involved in. It is recommended to prepare these materials in advance.

The sixth edition of the Fellowship will explore the theme of Empowering Meaningful Ageing through the Arts. It will focus on holistic arts-based approaches to healthy ageing, looking beyond traditional healthcare practices to explore how participation in the arts can enhance the well-being of seniors.

2.2. Please state the three latest examples of arts-based projects (in senior communities, where available) you have been involved in. Provide relevant websites of the projects/organisations and submit supporting documents from the listed examples.
