• Appropriate clothing and shoes should be worn to the K2 Summer Kamp
• Only staff is allowed to open the doors at the Summer Kamp (We want to make sure Kampers are safe at all times)
• No Kampers should leave out of the building without a staff (Supervision is mandatory)
• Please keep your hands and feet to yourself at all times
• Respect your peers, staff and yourself at all times
• Cursing, profanity or bad words will not be allowed at the K2 Summer Kamp
• Food or drinks are not allowed in the computer lab, classrooms or vans
• Please help keep the Summer Kamp building clean at all times
• Cellphones are not allowed to be used during K2 Summer Kamp time- Please put your phone on silence while in the training rooms (Classroom)
• Computer time is limited (Inappropriate computer usage is not allowed) Staff will give you privacy as well as monitor your computer use.
• NO MEDICINE ALLOWED IN THE K2 Summer Kamp building unless approved and given to the appropriate staff.
• NO WEAPONS ALLOWED IN THE K2 Summer Kamp building
• NO RUNNING IN THE K2 Summer Kamp building
• NO BULLYING, YELLING OR FIGHTING ALLOWED IN THE K2 Summer Kamp

2023 K2 Summer Kamp Registration Form

K2 Summer Kamp Dates: June 5 - August 4, 2023

I have voluntarily provided the above emergency contact information and authorize K2 Summer Kamp and its representatives to contact any of the above representatives on my behalf in the event of an emergency.

 

I give permission for K2 Summer Kamp to provide transportation to all community outings and special field trips while attending any session of the K2 Summer Kamp.

 

I give permission for K2 Summer Kamp to provide medical assistance in the event of an emergency. Employees of K2 Summer Kamp will not provide medical treatment beyond the level of First Aid and CPR Training.

 

K2Academy has installed electronic. digital cameras to record common area spaces and hallways forthe protection of all individuals. K2 Academy will clearly post notices in the locations in which the person has a reasonable expectation of privacy. K2 Academy will provide video information to the Agency for Persons with Disabilities or law enforcement in the event of any unlawful Level 2 Incident Report, or in the case of legal subpoenas or affidavits. K2 Academy accepts that it is unlawful to disclose, display, distribute or publish a photograph, videotape. film, or digital recording made in violation without the consent or knowledge of the person depicted. K2Academy is aware that it is unlawful for any person to knowingly photograph, videotape. film, digitally record or by any other means secretly view, with or without a device, another person without that person's consent under either of the following circumstances:

 

1. In a restroom, or other locations where the person has a reasonable expectation of privacy and the person is urinating. defecating, dressing. undressing, nude or involved in sexual intercourse or sexual contact.
2. In a manner that directly or indirectly captures or allows the viewing of the person's genitalia, buttock, or female breast, whether clothed or unclothed, that is not otherwise visible to the public.

 

I authorize K2 Academy use of photographing, videotaping, filming or digitally recording or viewing of my likeness, voice, and performance for surveillance/safety purposes only.

 

I authorize K2 Academy to provide photographing, videotaping, filming. or digital recording to authorized authorities. All footage can be used/not limited to misconduct concerns or incidents which occurred at the K2 Academy.

 

I hereby authorize K2 Summer Kamp to use pictures of me (or my child/ward) taken in a photograph, digital image, videotape, motion picture, and/or testimonial (written words The undersigned hereby releases K2 Summer Kamp, its agents or employees. as well as any and all users and exhibitors of said pictures, from any and all claims, demands, accountings, and causes for which the aforesaid videotape, testimonial, motion picture. digital image. or photograph likeness may be used pursuant to this Consent and General Release. If is also my understanding that I will receive no compensation for my likeness or testimonial.
