Please note that Dr. Ingram and her team review all requests and decide on engagement opportunities. Filling out this form does not guarantee Dr. Ingram's acceptance of the engagement request. If the engagement is accepted, you will be connected with Dr. Ingram's assistant to make all logistical arrangements and receive an invoice for a 50% honorarium.
This agreement is for the personal services of (Dr. Ingram) made between you(“purchaser") and Dr. Ingram for the engagement described.
I agree to fulfill the accommodations and honorarium provisions stipulated in this agreement, including the provision of 50% of the honorarium upfront and payment of the remaining balance on the day of the engagement.
I agree to uphold the standard of excellence during the engagement and throughout the entire process, including pre and post-engagement. It is essential that all parties involved maintain professional and informative while honoring partnering relationships when communicating with each other.
