This summer camp will take place at 1330 Castlemore Avenue, Markham, ON, L6E 1A4.

As part of the summer camp experience, 4 field trips are planned with the final 5th week for Olympic games
- 1st field trip is sponsored by MDI
- 3 field trips will be paid by parents

SUMMER CAMP 2023 PARTICIPANT WAIVER AND RELEASE OF LIABILITY

I, the undersigned, as the parent/legal guardian of the participant named above, acknowledge and understand that participation in the Summer Camp 2023 ("the Camp") hosted by the Islamic Centre of Markham ("the Centre") involves certain risks, which may include, but are not limited to, physical injury, illness, or accident.

In consideration of the opportunity afforded to my child to participate in the Camp, I, on behalf of myself and my child, hereby release, discharge, and agree to hold harmless the Centre, its employees, volunteers, representatives, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or death, that may be sustained by my child while participating in the Camp or while in, on, or upon the premises where the Camp is being conducted.

I further agree to indemnify and hold harmless the Centre, its employees, volunteers, representatives, and agents from any loss, liability, damage, or costs, including court costs and attorney's fees, that they may incur due to my child's participation in the Camp, whether caused by the negligence of the Centre or otherwise.
