    All new guests start here!  This form allows me to get to know a little bit about you and your hair goals so that I can ensure your initial appointment is booked properly. I can also begin forming a plan to ensure your first appointment is a successful one! Upon completion, you'll be given the option of a link to book your appointment, an in-person consultation, or for me to recommend the best service session for you, based on your information.   • By completing this form, I am submitting an appointment request or appointment recommendation request. In response, I will receive an email and/or text message notifying me whether my appointment request was confirmed or denied, or an email with a link to book my recommended appointment. 


    Once approved, my appointment in-salon will begin with a 15 minute full consultation to refine the plan for service, based on my digital consultation form.


    Generally, new guests are able to be seen within 1-3 weeks of appointment submission request, but guests with limited availability may have to wait up to five weeks or longer to be seen.

     

    I have read and understand the following Booking and Cancellation policies:

    • I may book for New Guest sessions, New Guest consultation, or patch test only. If I book any Loyal Guest session or individual services, my appointment will be denied.
    • I will be charged a booking fee of 50% of the scheduled session/service total, and this fee will be applied in full to my session/service total on the day of my appointment.
    • I will receive a courtesy text and/or email reminder approximately 24 hours before my scheduled appointment, but am soley responsible for attending my requested and scheduled appointment. 

     

    Any cancellations, rescheduling, or changes to my apointment must be done 24 hours or more before my scheduled appointment time.

     

    • If I need to cancel or reschedule my appointment, or change/reduce services, I must do so online, via the link sent in my confirmation text/email, or through the PocketSuite app.
    • If I need to reschedule or change/reduce services, I must cancel the original appointment and book a new appointment.
    • If I cancel and reschedule, my booking fee is transferable to one (1) rescheduled appointment. If I cancel the rescheduled appointment (whether in the sufficient time frame or not), I forfeit my booking fee and must pay the booking fee again for any subsequent appointment.
    • If I cancel and choose not to reschedule, my booking fee will be refunded in full- as long as I have canceled 24 hours or more before my scheduled appointment start time.
    • If I cancel my service or consultation appointment with less than 24 hours notice, I forfeit my booking fee.
    • If I no-call/no-show, I forfeit my booking fee and will be prohibited from booking future appointments with the salon.

     

    I understand that this and other salon policies are available to view on the FAQ page at www.azaleacolorbar.com, as well as on the booking page, and that I will be required to sign my acknowledgement of these policies upon booking.
