Consent for Use of Photograph/Video

 

By signing this Photo Release Form, I hereby grant Empowering Young People the absolute and irrevocable right and permission, with respect to any photographs, video recordings, or digital images (collectively referred to as “Media”) taken of me, my child(ren), or those under my guardianship, to use, reuse, publish, and republish the Media:

1. Purpose of Use:

• For promotional, marketing, educational, or other lawful purposes.

• In any medium including, but not limited to, print, social media, websites, and other digital platforms.

2. Rights Waived:

• I understand that I will not receive any monetary compensation or royalties for the use of the Media.

• I waive any rights to inspect or approve the final version of the Media, including any written copy associated with it.

3. Ownership and Usage:

• I acknowledge that all rights to the Media, including copyright, belong to Empowering Young People, and that they have the sole discretion to use, edit, or dispose of the Media as they see fit.

4. Release from Liability:

• I release and discharge Empowering Young People, its employees, agents, and representatives from any and all claims, demands, or causes of action that may arise from the use of the Media, including claims of defamation or invasion of privacy.

 

By signing below, I confirm that:

• I am at least 18 years old or am the legal guardian of the minor(s) in the Media.

• I have read and understood the terms of this release and agree to be bound by them.

I certify that all information on this application is accurate and complete to the best of my knowledge. I understand that should this application contain any false or misleading information, my application may be rejected, or my volunteer position terminated. I hereby give permission to fully evaluate my past employment, education, background, and social media activity. Finally, I release all persons, companies, or organizations from liability for gathering or supplying the requested information.
