  • STATEMENT OF RELEASE

  • Ihear by authorize the leadership of Oak Pointe Church to make any and all inquiries regarding my application and to verify all information contained in this application for a volunteer position in Oak Pointe Student Ministries. I also grant permission for any representative of Oak Pointe Church to share any information they believe may impact a decision for the volunteer position am applying for. I hereby authorize any individuals to disclose any and all information to Oak Pointe Church. I release all such persons or entities from liability that may result or arise from Oak Pointe Church's collection of all information for its consideration of my application. This release is in effect for 90 days from the date I submit my application to the Oak Pointe Student Ministries Staff. I declare under penalty of perjury, under the laws of the State of Michigan that the foregoing is true and correct. I hereby request the Michigan State Police to release any information which pertains to any record of convictions contained in its files or in any criminal file maintained on me whether local, state or national. I hereby release said police department from any and all liability resulting from such disclosure.
