PHOTO, AUDIO, ELECTRONIC MEDIA RELEASE FORM FOR MINORS. As parent/guardian, I understand that promotional pictures and videos (individual and group) may be taken during Faith Formation Classes. I give permission for my child(ren)'s (named in this registration) pictures to be used for church promotional materials such as newsletters, web pages, calendars, power point presentations, or videos to promote or highlight these classes or activities. I further grant permission for the resulting photographs and/or videotaped footage to be edited, if necessary, and then published for the purpose of promoting the ministry work of the parish.*

Virtus "Teaching Touching Safety" Program Archdiocese of Los Angeles Holy Family Faith Formation Program will present a sexual abuse prevention program, “Teaching Touching Safety.” The creators of the Protecting God’s Children™ program developed the Teaching Touching Safety program. This program provided to us by the Archdiocese of Los Angeles, and as part of our ongoing effort to help create and maintain a safe environment for children and to protect all children from sexual abuse. As a parent, you have the right to choose whether your child participates. We encourage you read the Protecting God’s Children ® Teaching Touching Safety- A guide for Parents, Guardians and Other Caring Adults found on their website www.virtus.org so you will be aware of the nature of this important program. For more information visit the VIRTUS online website www.virtus.org.I am allowing my Child to participate in the Protecting Gods Children “Touching Safety program” and am specifically requesting that Holy Family Faith Formation For Children present the program to my child whose name is listed below.*
