Parental Consent

 I hereby grant West Meade Fellowship permission to photograph my child for use of electronic media, video or photographic images of any kind.  I understand and agree to these pictures being used as needed as the church deems fit. This may include but is not limited to social media, fundraising events, press releases, printed media, etc.
