Relative Providers must have been receiving a subsidy payment from Caddo Nation on or before March 11, 2021.

Center/Home Based Providers must be licensed/regulated/registered/certified on or before March 11, 2021.

Subgrant awards are based off of a base amount and the quality rating. Relative Providers award amounts are based off of a base amount with bonuses for indigenous lesson plans and indigenous meal plans. 

Subgrant award periods are for six months. After that time, providers may apply for additional funding as long as funds are available. The Caddo Nation Child Care requires a mid-term and final report which will be provided to grantees. 

Additionally, it's important to note, this funding is taxable and it is your responsibility to consult with your tax professional.

Note: Only upload a W-9 if you are a new vendor for Caddo Nation. 

Tribal lead agencies are required to limit ARP Act Stabilization subgrant applications to providers serving Indian children.

Child Care providers that are receiving stabilization subgrants from a tribal lead agency must be serving at least one Indian child but are not restricted from recieving stabilization subgrants from a tribal lead agency if they also serve non-Indian children or have received a stabilization subgrant from a state. 

Subgrant funds my only be used for the following categories. Because this is an estimate, you can move funds between categories without prior approval. You may choose to use funds for one or more of the following categories; please check the appropriate box(es) and enter the estimated monthly amount or total of reimbursable receipts per category. If you do not anticipate spending funds in an expense category, please leave it blank.

To receive a stabilization grant, I agree to use the funds only for the categories and purposes indicated on this application and have marked above which categories I plan to fund. Note: You can move funds between categories without prior approval.

 

I also understand that it is my responsibility to maintain records, receipts, and other documentation to support the use of funds I receive, as well as to document my compliance with the requirements described in A, B, and C.

 

By signing this application, I am certifying that I will meet requirements throughout the period of the subgrant, including the following:

 

1. When open and providing services, I will implement policies in line with guidance and orders from corresponding state, territorial, Tribal and local authorities and, to the greatest extent possible, implement policies in line with guidance from the U.S. Centers for Disease Control and Prevention (CDC).
2. For each employee (including lead teachers, aides, and any other staff who are employed by the child care provider to work in transportation, food preparation, or other type of service), I must continue to paying at least the same amount of weekly wages and maintain the same benefits (such as health insurance and retirement) for the duration of the subgrant. I understand that I may not furlough employees from the date of application submission through the duration of the subgrant period.
3. I will provide relief from copayments and tuition payments for the families enrolled in the child care program, to the extent possible, and prioritize such relief for families struggling to make either type of payment.

Provider Affirmation

The following signature affirms that I will adhere to the items noted in A, B, and C. It also affirms I will only use the funds in the area noted in Section 5 of this application. In accordance with CCDF guidelines of this grant, I will not use these funds to reimburse any costs prior to January 31, 2020 (declaration of the public health emergency). I understand that reimbursement of funds are only for use of the eligible grant categories outlined in Section 5, as long as the purchase was in response to the COVID-19 public health emergency.

 

By signing below, you certify all information included with this application is true and correct to the best of your knowledge. You are in good standing with Oklahoma's regulations for licensed or registered care. You also certify that you have been negatively impacted by the COVID-19 public health emergency and any funding or assistance received will be used for the purpose authorized in the Child Care Stabilization Program Guidelines.

In order to process payments efficiently, the Caddo Nation strongly recommends that Providers receive payments through direct deposit. Attached to this application is an Electronic Funds Transfer Form. Complete the form and attach either a voided check, blank deposit slip, or a letter from your financial institution.

 

The Caddo Nation will only process checks by special exemption made by Caddo Nation Child Care Staff. Providers will have to come to the Caddo Nation during office hours to sign for the payment unless otherwise arranged. Checks will not be mailed due to the large amount.

Instruction to Payee

1. The ACH Enrollment Form and Agreement allows Caddo Nation of Oklahoma's Accounts Payable Department to automatically deposit payments directly into a finanical account of a vendor. 
2. Complete the form.
3. Sign the form which will go to Caddo Nation of Oklahoma for processing. 
4. All accounts will be prenoted (tested) to ensure the information provided is valid. 
5. Incorrectly completed forms may cause significant delays in establishing direct deposit payments.

Terms and Conditions

1. Payee/Vender agrees to accept payment by Caddo Nation of Oklahoma through electronic funds transfer (EFT) and that Caddo Nation of Oklahoma can rely on the information provided on this form.
2. This agreement applies to and amends all agreements with Caddo Nation of Oklahoma by incorporating these terms and condititions neither enlarge nor diminish the respective rights and obligations contained in any other agreement with you. The payment terms are not affected. Caddo Nation of Oklahoma will consider payment received by you when your finaicnl institution has received or has control of the payment. 
3. Payments to you will be deposited into the account designated on the front of this ACH Enrollment Form and Agreement until Caddo Nation of Oklahoma is notified in writing that you wish to cancel this authorization or designate a different financial institution or account. A minimum of ten banking days will be needed to execute your instructions.
4. Caddo Nation of Oklahoma has the right to adjust future payments if previously made are found to be duplicates, in ecess of requirements, fraudulent, in error, or require any other adjustment under the terms of an agreement with you. This may be accomplished by using an ACH debit.
5. Caddo Nation of Oklahoma will not be responsible for any loss arising solely from error, mistake, or fraud regarding information on your ACH Enrollment Form and Agreement. 

I herby authorize Caddo Nation of Oklahoma to automatically deposit payments to the account listed above under the terms and conditions of this ACH Enrollment Form and Agreement. I certify that I am authorized to enter into this agreement on behalf of the account holder. I verify that the information provided on this form is correct.
