LOST/DAMAGED/MISSING PPE
Use this form to report lost or damaged PPE, provide details, and request repalcement PPE.

BODY HARNESS and LANYARD
For Linemen, a body harness is required. The harness is worn snugly and a lanyard arttaches to the anchor on at the bottom of the bucket.
