In addition to the application above, proof of certification and proof of payment is required to process reimbursement payments. Your certifier may provide DATCP with supporting documentation, but this needs to be arranged between you and your certifier.
Please note that cancelled checks or invoices that do not show proof of payment are not an acceptable form of documentation.
