1. I will complete all necessary forms (medical, consent, etc before my child attends the SACC program.
2. I will notify SACC of any changes in registration information (address, phone number, email, etc.
3. I understand I am responsible for paying program fees which must be submitted electronically. All days scheduled must be paid prior to attendance each month. Paying in advance secures your child's spot, failure to pay in advance and a delinquent account greater than one month's dues could result in a pause or removal from the program. Please call Kyle Davis if you have any questions regarding payment.
4. I understand if a child is not picked up by 5:59 PM I will be charged a fee of $20. If your child is not picked up by 6:30, and attempts have been made to contact guardians parents and emergency contacts, the Columbus Police Department will be contacted. Three or more recurrent late pickups are subject to removal from the program.
5. Upon the arrival of the guardian, you/they are responsible for your child. This includes on the playground, re-entering the building, etc. Once you arrive, you must depart with your child or exit your vehicle and remain with your child.
6. I will make other arrangements for emergency closings and early dismissals due to weather or facility emergencies. I understand that the SACC program at OLP will not be in session when Our Lady of Peace and Columbus Catholic Schools are closed. SACC will not be open when OLP makes an early release, including the final day of school.
7. I will inform SACC in writing if I do not want photographs of my child participating in the program to appear in any SACC media publications.
8. I agree to read the Our Lady of Peace Parent/Student Handbook, which contains detailed information regarding the SACC program. I will also read the SACC update emails that may mention changes in policies, schedule, etc.
9. I understand guardians typically should not enter the building. Guardians will be given a phone number to call when you arrive to pick up your child. This number will connect you directly to a SACC staff member. We will dismiss the child out the west door of the school. (Same as morning drop-off)
10. Anyone picking up your child for the first time must present a photo ID, and must be on the list you provide of people to whom SACC is authorized to release your child or have emailed permission.
11. I understand that I must inform the director each day/time that my child is scheduled to attend SACC but will not be present by email. I understand failing to do so will result in the program halting until confirmation of the child's whereabouts. I recognize the program is run by the Church, and will not always be able to communicate with school staff about absences.
