    SUBMISSIONS DUE BY 5PM FRIDAY 9TH JUNE

  • Service awards shall be awarded for:  (a) 5 Year Service Award – Certificate  (b) 10 Year Service Award – Badge. For the Executive to grant an award, the nominee should have served District swimming for the corresponding number of years as:   (a) District Administration or Official; or  (b) District Competition official; or  (c) District Delegate; or  (d) Have other service the Executive may consider comparable.  The standard or level of service required is the regular performance of the function or office(s) over the prescribed period (eg. service as a competition official requires regular attendance at competitions). Service Awards can be applied for by oneself, or may be nominated by another individual.

  • Submission of this application does not guarantee the recipient of a service award. Applications will be assessed against the criteria to ensure that the applicant has fulfilled all requirements to receive a Metro West Service Award.
