The Church conducts private religious practices that include the use of entheogens as sacrament, including but not limited to, use for the purpose of spiritual enlightenment, spiritual healing, personal spiritual growth and consciousness expansion. Whereas the member desires to voluntarily participate in such practices and to join the Church in order to engage in religious activities and experiences. Member is aware that the Church conducts private religious practices that include the use of entheogens as sacrament, including but not limited to, use for the purpose of spiritual enlightenment, spiritual healing, personal spiritual growth and consciousness expansion, with the ultimate goal of spiritual enlightenment.
The member wishes to participate in such practices and is joining the members only private church to engage solely in religious and spiritual activities. Member signs this agreement under penalty of perjury, and affirms that there is no other reason beyond participating in religious, spiritual and church services and sacrament, that they are joining the church.
Joining Member agrees as follows:
Member does not work for, nor are they acting as an agent for any other party who has requested them to join the Church for investigative or law enforcement purposes. By signing this agreement the member waives thier qualified immunity.
Member is joining only for their own spiritual or religious exploration and is familiar with the effects of the sacrament they are choosing to use.
Use of Entheogens: Member acknowledges that certain religious practices of the Church involve the use of entheogens as sacrament, which are substances that can induce spiritual or mystical experiences, and altered states of consciousness in order to achieve enlightenment. Member agrees to participate in these practices responsibly, and in accordance with all applicable Church guidelines and any safety instructions given by Church leaders. Member is 21 years of age or older and is legally competent, and is voluntarily entering into this agreement with a full understanding of its terms.
Member agrees not provide sacrament given to them by the Church to non Church members. All funds donated by Members will be used for Church operations, future sacrament and ceremonies. All donations are non refundable.
Members agree to be safe and use any sacrament for religious and spiritual development towards the path of enlightenment.
Member agrees to disclose to the church any medical, psychiatric or psychological conditions they may have and unless disclosed, Member attests they do not have any serious physical health ailments that would contradict with their use of sacrament. They have discussed their use with their health care providers and are aware and responsible for any risk they voluntarily are taking if they choose to as a Member use or be around sacramental use ceremonies. As such they agree to hold church or its agents or affiliates harmless for any accident resulting from said use or presence.
Member will disclose to the Church any medical, psychiatric, or psychological conditions that could potentially make the use of entheogens unsafe. The Church may, at its discretion, deny membership to anyone it believes may be at risk.
Member will keep all church practices, information, teachings, and ceremonies confidential, unless express permission to disclose is granted in writing by the Church. Furthermore, Member will not reveal the identity of any other Members of the church. Members are free to talk about their own personal religious experiences.
Waiver and Release:
The Member voluntarily assumes all risks associated with participation in Church activities, including but not limited to the use of entheogens. The Member agrees to release, indemnify, and hold harmless the Church and its leaders from any and all liability related to the Member's participation. This Agreement will be governed by and construed in accordance with the laws of the jurisdiction where the Church is located. If you are a law enforcement officer you agree to waive your qualified immunities otherwise that you would be entitled and accept personal responsibility for violating the confidences of the church and will be responsible for the legal fees stemming from any legal proceedings held against the church or its members including criminal proceedings for church sacramental related activities.
Member signs this agreement under penalty of perjury that statements are true to the best of their knowledge at the time of signing.
MEMBER RIGHTS Church grants members the rights and privileges of membership in accordance with the policies and procedures which the church reserves the right to amend at any time in writing. Church may establish its own procedures and may exclude or remove members by vote of recognized enlightened members of the church. Members with ordained enlightenment status for the church. The Church may, at its discretion, deny membership to anyone.
