Music Together Class Registration

Please fill in all information below to register your child for Music Together. Each class has a capacity of 12 children; classes are first come, first served for children ages 0-5.

The class is held on Saturdays from 10-10:45am at the Community Concert Center in Naperville (104 E. Benton Avenue).
If you have more are registering more than one child, please fill out this form for each one.
Babies 8 months and under are free with a registered sibling. Please include their information below.

I give Young Naperville Singers (YNS) permission to publish and reproduce in YNS publications and promotional materials, in print, electronic, digital or video format, the likeness or image of my child without further consideration or compensation related to the use of the likeness or image (names will not be published). I understand that YNS will have the right to crop or treat the likeness at its discretion. I further understand that YNS has established an internet website and social network, and the likeness or image of my child may be included on those sites and can be downloaded by any computer user. I hereby forever release and will hold harmless YNS, including, but not limited to its Board of Directors, Advisory Board, employed staff and volunteers, from any and all claims of any kind, nature and description which may arise as a result of the publication or reproduction by YNS, or any third party, of my child's likeness as provided in this agreement.
