Contract Agreement
By submitting this form, I am in agreement to the terms below. I am committing to attend all ten Modules to receive the discounted scholarship rate of $6500, instead of the normal rate of $11,950, with no interest accrued. I agree to pay an initial deposit in the amount of $1000 at this time to secure my scholarship. An additional $1500 will be due two weeks before the first Module. I understand that when I pay the initial deposit, I must save a credit card on file and I agree to allow that card to be used for all remaining payments. The payments will begin the month of the first Module that I attend and will be automatically charged on the 25th of each month, consecutively, until paid in full. My payment plan for the remaining $4000 is to pay $160/month for 25 months. I agree to pay $10/month for any month that my credit card is not saved or is declined, to cover the additional work that must be performed by the office. By accepting this scholarship, I understand I am taking this opportunity away from other applicants and it is therefore binding. If I choose to leave the program, I am not entitled to any refund or relief from the committed $6500 amount due but will be allowed to complete any missed modules at the next available date. I acknowledge that I am healthy and am in good standing to participate in this workshop. If my health changes, I will contact the office. I understand that to become an Authorized Continuum Movement Teacher, I am required to sign a yearly contract, pay yearly dues, and provide proof of Insurance with Continuum Movement listed as additional insured.
