Anchor Point 3D Archery Sign-up

This form is used to book a spot to shoot Dave Burridge's 3D Archery course.
Please read all the information below before signing up.
- Location - The course is located at 722 Davis Dr, Uxbridge, ON L9P 1R2, Canada (map)
- Pricing - The cost to shoot is $40 per person. However, if cost is an issue, please contact Dave (dburridge@yugta.ca) to discuss this further. We promise we will do our best to work something out.
- Deposit - There is a non-refundable $40 deposit for you to confirm your booking. Please carefully read the deposit section of this form below before proceeding.
- Waivers - Each person in the group must bring a signed waiver on the day of shooting the course. This waiver can also be filled out electronically and emailed to Dave (dburridge@yugta.ca). Waivers can be downloaded below.
  - Under 18 waiver (to be signed by parent/guardian) download here
  - Adult (18+) waiver download here
- Eye Dominance Test - To speed things up, please have each participant watch this video to determine their dominant eye.
- Bring - In order to help you all to have the best possible experience, please have each participant bring the following:
  - bugspray
  - sunscreen
  - water & snacks
  - boots (if you have them, it can be mucky)
    
    If not boots, then closed toe shoes

Deposit

There is a non-refundable deposit of $40 to confirm your booking. Since there as there are limited dates during the year when archery can be run, we encourage everyone to be considerate to both Dave and other potential participants, minimizing your chances of last minute cancellations. If you cannot make your booked time slot for whatever reason, please let us know and we will reschedule the booking so another group may have your spot. If you have any questions or concerns, please contact Dave (dburridge@yugta.ca).

Please choose the date you would like to come and shoot the course. Please note, if there is a weekend date you would like but it is not available, feel free to reach out to Dave (dburridge@yugta.ca) to see if it is possible to work something out. There are a few dates not on the calendar selection which are available in the additional dates section - if you cannot find a date here that suites your availability, please feel free to look at the additional dates.
