Welcome to Church of the Ascension! Please complete and submit the parish registration form below. We are excited to welcome you to the Church of the Ascension community. Please help us serve you better by filling out all of the relevant items with information for yourself and then for each individual you are registering as part of your Household. If you were a member at some point in the past and are re-joining our parish, it is likely that you do not need to fill out this registration form. Please call the parish office and we can update your information and make you active again in our database. If you are a young adult now and were registered with your family as a child, you do not need to fill out a new registration form. We will update your information when you call the parish office. Be sure to fill in your registration form completely with your address, email and especially birthdates of your children. This will help us to notify you when it is time for you to register your children for sacraments. Also, please do not put in duplicate emails or phone numbers. No need to put in the same ones for each member. The form should be completed once for each member of your household, including spouse, children, and any additional member of your household interested in joining us. Use the same email for the primary contact of the household (1st question). Be sure to click the 'Submit' button and contact the Parish Office (612-529-9684) with questions.
