  • Grant Proposal Routing Form

  • Please submit this form at least one (1) calendar week before the sponsor's submission deadline or intended submission date. Attach/submit the following documents noted if you have not previously shared them (attachments/links may be added at the end of the form).

    1) Draft project/research narrative (or statement of work if for a subaward);

    2) final budget and budget justification;*

    3) sponsor forms, letters, etc. requiring signature by a college administrator; and 

    4) NSF and PHS-specific forms.

    (a) If applying to the National Science Foundation or a Public Health Services sponsor (e.g., National Institutes of Health), all investigators must submit a Disclosure of Significant Financial Interest form.

    (b) If applying to the NIH, all PIs must submit a PI Assurance Certification.

     *Please contact Aubri Drake to begin drafting a budget and justification as soon as possible. The budget should be final one (1) calendar week before the submission deadline.

  • PROJECT DIRECTOR/INVESTIGATOR CERTIFICATIONS:

  • Debarment/Suspension: The Principal Investigator and co-investigators are not debarred or suspended from doing business with the federal government.

    Human Subjects Training: For projects involving human subjects, the Principal Investigator and co-investigators certify that they have completed or will complete all required human subjects training.

    Financial Conflict of Interest: If the grant seeks funding from Public Health Service agencies (e.g., NIH) or NSF, all investigators must complete a Disclosure of Significant Financial Interest. 

    Financial Conflict of Interest Training: All investigators seeking or in receipt of Public Health Service funding certify that they have completed or will complete the required financial conflict of interest training.

    NIH Proposals: The Principal Investigator(s) must complete a NIH PI Assurance Certification.

    NSF Proposals (effective July 31, 2023): Principal Investigators, other senior personnel, undergraduates, graduate students, and postdoctoral researchers must complete RCR training before engaging in research. Training is available through the online CITI Program.

  • Provide the email address of the PI's department head/chair so that the form may be routed to them for approval.

    Proposals are reviewed and must be approved by the Associate Provost and the Senior Academic Budget Manager, or delegatee(s), before submission to a sponsor or funding organization.
