RECREATIONAL PRIVATE LESSON REQUEST FORM
Payments are due in full once Private Lessons have been scheduled and prior to the first scheduled private lesson. CASH payments are preferred. 3% fee will be added to credit/debit card payments.

Cancellations/rescheduling must be made a minimum of 24 hours in advance OR PAYMENT WILL BE FORFEITED AND INSTRUCTOR WILL BE PAID. We must have ample time to reach the instructor before he/she arrives to teach your child.

Buddy Single- 2 or 3 students at similar level. 60 minutes: ($45 per member/ $50 non member)

Buddy 3 Pack - 2 or 3 students. Similar level. 3 times. 60 min. Same coach. Within 3 weeks ($125 per student)
