Has the patient been diagnosed with Osteoporosis or low bone mineral density? Ie. T score -2.5 or lower, or Z score -2.0 or lower.
Has the patient ever been on medication for Osteoporosis? Ie. Alendronate (Fosamax), Risedronate (Actonel), Zoledronate infusion (Aclasta), Denosumab injection (Prolia) or Romosozumab injection (Evenity).
Please provide medication details ie. Name of medication, when commenced or ceased and if the type of medication has changed since the last BMD.
Has the patient commenced, ceased or changed the type of medication for Osteoporosis SINCE their last BMD?
Is the patient currently using any oral or inhaled steroid, glucocorticoid or cortisone medication (7.5mg Prednisolone or similar) for any medical condition?
Please provide the medication name, dose and duration of course.
Does the patient have an adrenal or pituitary gland disorder that causes production of excess cortisol? Eg. Tumours, Cushing's Syndrome.

Important note: ALL form questions must be completed for an accurate result. If there are no item codes listed below then the patient is not currently eligible under any rebateable item code (use code BMDNR; No Rebate).
- Eligible for item code 12320 if the patient has never had a BMD, and
  
  The patient is 70 years of age or over
- Eligible for item code 12320 if the patient has NOT had a BMD within the last 5 years and 1 day, and
  (a) The patient is 70 years of age or over, and
  
  (b) The t-score for the patient's last bone mineral density is -1.5 or more
- Eligible for item code 12322 if the patient has NOT had a BMD within the last 2 years and 1 day, and
  
  (a) the patient is 70 years of age or over; and
  (b) the t‑score for the patient's last bone mineral density is less than ‑1.5 but more than ‑2.5
- Eligible for item code 12306 if the patient has NOT had a BMD within the last 2 years and 1 day, and
  
  (a) has one or more fractures occurring after minimal trauma; or
  
  (b) has been diagnosed with Osteoporosis proven by bone densitometry at least 12 months previously
- Eligible for item code 12321 if the patient has NOT had a BMD within the last 1 year and 1 day, and
  
  Has had a significant change in therapy for diagnosed Osteoporosis (Ceased, commenced or changed type of medication for Osteoporosis)
- Eligible for item code 12312 if the patient has NOT had a BMD within the last 1 year and 1 day, and one or more of the following:
  
  (a) prolonged glucocorticoid therapy;
  
  (b) any condition associated with excess glucocorticoid secretion;
  
  (c) male hypogonadism;
  
  (d) female hypogonadism lasting more than 6 months before the age of 45
- Eligible for item code 12315 if the patient has NOT had a BMD within the last 2 years and 1 day, and one or more of the following:
  
  (a) primary hyperparathyroidism;
  
  (b) chronic liver disease;
  
  (c) chronic renal disease;
  
  (d) any proven malabsorptive disorder;
  
  (e) rheumatoid arthritis;
  
  (f) any condition associated with thyroxine excess
