Apollo Talent Show Entry Form

By signing and submitting this form, you agree and will adhere to the Talent Show Rules and Regulations that is attached to this and you may be subject to disqualification if any are not abided by.

 

If you like to attend the event please sign up and fill out the Accident Waiver & Release of Liability form that is attached to this.

 

If you are under the age of 18 your parents/guardian have to sign.

Talent Show Rules and Regulations

 

1.  Acts requiring more than 12 FEET of ceiling height will not be allowed.

 

2.  Acts will be allotted no more than two (2) Minutes to perform. Acts will be PENALIZED if time is exceeded.

 

3.  Participants may only perform ONE time during the show. Any participant who participates in more than one performance will disqualify from BOTH acts.

 

4.  NO profanity or vulgar lyrics in any vocal performance or dance music.  This is a church and family show!!!

 

5.  NO nudity allowed! Keep attire at a respectable and tasteful level.  NO alcohol, tobacco, or drug advertisements are to be shown on any clothing.

 

6.  NO pyrotechnics, open flames, or any other dangerous additions.

 

7.  A Sound Technician and microphone(s) will be provided. Acts MUST provide their own recorded music, instruments, props, etc. if needed. Recorded music MUST be on a CD or USB flash drive in an acceptable format (i.e. MP3 or WAV file).

 

8.  EVERY participant should attend one interest meeting.

 

9.  EVERY participant MUST attend the rehearsal. Acts requiring recorded music MUST bring it to rehearsal. NO changes to the music selection will be accepted after rehearsal!

 

10. If a group wins they have to slip the prize.

 

 NO EXCEPTIONS WILL BE MADE TO THESE RULES!

 

Rules are subject to change at the sole discretion of the Talent Show organizers.

 12.1   Accident Waiver and Release of Liability

 
In consideration of my application and permitting me to participate in any Community Missionary Baptist Church event, I hereby take action for myself, and assign as follows:

 

(1) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities of persons released, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from event(s), THE FOLLOWING ENTITIES OR PERSONS: Community Missionary Baptist Church. and/or their directors, officers, employees, volunteers, representatives, clients, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers;

 

(2) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise.

 

I acknowledge that Community Missionary Baptist Church. and its directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific event or activity on behalf of any other company, business, or organization.

 

The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

 

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

  

12.2   Media Release

 
Community Missionary Baptist Church periodically uses photographs, videos, audio footage, or testimonials of program participants for many purposes.  By my signature on this consent form, I grant Community Missionary Baptist Church, its representatives, and employees the right to take photographs/videos of me in connection with the organization’s functions.  I authorize Community Missionary Baptist Church, its assignees, and transferees to copyright, use and publish the same in print and/or electronically.

 

I agree that Community Missionary Baptist Church may use such photographs/videos of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
