Please complete the logo release form below to use CAUCQ Program logos including Accreditation, Qualification, Certification and Commendation Program logos. After submission, you will receive the logo files via email.

Your organization will be authorized to copy, exhibit, distribute and publish in print, digital, and/or video productions the requested material for purposes of publicizing your participating with a CAUCQ Program. Only organizations that are currently active and in good standing with the program may use the programs’ appropriate logos.
