    Below is a list of the different Skin Types - please read through each and identify which skin type is yours:

  • Have you ever had skin cancer?*
  • Has anyone in your immediate family ever had Melanoma Skin Cancer?*
  • Has anyone in your immediate family ever had Melanoma Skin Cancer?*
  • Do you have a medical condition that is made worse by sunlight?*
  • Do you have a history of severe sunburn, especially in childhood?*
  • Do you have an excessive number of moles and/or freckles that cover over 60% of your body?*
  • Are you pregnant?*
  • Are you currently on any medication which increases the skin’s sensitivity to UV light?*
  • Have you undergone any form of surgery in the last 6 weeks?*

  • If you have answered ‘YES’ to any of the above you should NOT use a sunbed.

  • ACKNOWLEDGEMENT AND AGREEMENT:

    • I AGREE to you maintaining the information above to allow me to use your sunbeds in conjunction with the General Data Protection Regulations 2018 (GDPR)
    • I have read and understood the information and terms and conditions provided and agree that my data will be processed per TruTan's privacy policy.
    • I have given accurate answers to all the questions and I will consult my GP where necessary.
    • I undertake to advise the salon if my medical circumstances change.
    • I have been given adequate instructions for the proper use of the tanning equipment, understand the risks involved, and use it at my own risk.
    • I confirm that I am over 18 years of age and I am suitable to use UV tanning equipment.
    • I hereby agree to release the owners, operators and manufacturers from any liability resulting from use of the tanning beds.
