English Introduction

感謝你對《生命恩泉》的支持和有興趣成為我們的義工。為了更好地組織和關顧我們的義工團隊，我們特此設計了這個登記表格。請根據你的情況選擇適用的選項：

1. 首次申請的義工：如果你尚未成為《生命恩泉》的義工，請填寫表格並提交你的資料。在確認收到你的資料後，我們將聯絡你並安排一個面談時間。目的是希望能更深入地了解你的興趣、技能和服務意願，以便我們能更好地安排你參與我們的福傳事工。

 

2. 現任義工和重新投入服務的義工：如果你已經是我們的義工，或者曾經是我們的義工，現希望重新投入服務，請填寫表格並提交你的資料。你將收到一封確認電郵，其中包含你的義工編號。請妥善保存這個義工編號，它將作為辨識你身份和參與事工的重要標識，有助於我們管理義工的資料，協調服務時的安排，以及提供更有效的溝通和支援。

本人明白此等資料將保持機密，並屬 《生命恩泉》 所有。
同時，本人明白本人的聯絡資料將提供予相關的小組組長/協調員，以便其與本人聯絡。
如獲選擔任義工職位，本人同意對在工作中接觸到有關 FLL、其員工及義工的事務資料嚴格保密，法律另有規定除外。

 

I certify that the information provided on the Volunteer Application Form is true and complete. 
I understand that this information will remain confidential and is property of FLL .
As well, I understand that my contact information will be given to the appropriate Team lead/ Coordinator so that he/she may contact me.
If selected for a volunteer position, I agree to keep confidential any information that I may come across regarding the affairs of FLL, its staff and volunteers, unless otherwise directed by law.

Due to the processing time within our system, we kindly ask for your patience as we proceed with further follow-up and communication. Additionally, we are currently actively enhancing our volunteer management system, which may require additional information from you. We anticipate meeting with you in the near future.

Between

Fountain of Love and Life (hereinafter “FLL”)

And

As a volunteer with FLL, I agree as follows:

TERM

The volunteer term shall begin on the date this Agreement is signed and shall continue for the duration of the Volunteer's service with FLL. I further understand that either I, or FLL may terminate this agreement, ending my volunteer duties, at any time and for any reason, without notice and without providing compensation.

VOLUNTARY WORK

I acknowledge that all the work that I perform is on a volunteer basis and there is no employment relationship with FLL. I understand that I am NOT entitled to receive a salary, remuneration, or any other employee benefits from FLL, including health or medical insurance benefits.

SUPERVISION

I agree to work under a supervisor’s direction, as assigned from time to time.

SAFETY

I understand that I have a right to refuse any work that is unsafe as per the Occupational Health & Safety Act.

ASSUMED RISK

I acknowledge that there may be risks associated with or related to the volunteer duties I may be assigned including any manner of injury from use or misuse of equipment/tools required to perform my volunteer duties.

CONFIDENTIALITY

FLL is obliged to ensure that only those personal data that are necessary for effective management and coordination of volunteers are collected, and all the collected personal data will be handled in strict confidentiality. 

I acknowledge and agree that any information I gain in the course of my volunteer duties is considered confidential. I further agree not to disclose any such information to anyone for any reason and that this obligation extends beyond the termination of my volunteer duties.

DIRECTION AND GUIDANCE 

FLL agrees to provide necessary and practical direction and guidance, as well as training if needed, for a volunteer to perform duties effectively.

POLICIES OF FLL

I understand that there are various policies in place that apply to FLL, including all staff and volunteers. I agree to abide by them all. A copy of the FLL Code of Conduct and the FLL Screening Policy have been provided to me and is available on the FLL website.

RELEASE AND WAIVER

The undersigned volunteer and FLL release and hold harmless each other and their directors, governors, executive, employees, agents, heirs, executors, administrators, successors and assigns from any and all liability for any loss, damage, injury and/or expense that they or their next of kin may suffer as a result of the work of the undersigned volunteer, including but not limited to accidents, acts of God, war, civil unrest, sickness, transportation, scheduling, government restrictions or regulations, and any and all expenses which may be incurred because of said work, including without limitation any loss, claims, damages, expenses, judgments, or attorney fees for which FLL and the undersigned volunteer is held legally liable.

The undersigned volunteer and FLL further release and hold harmless each other and their directors, governors, executives, employees, agents, heirs, administrators, successors and assigns from any and all claims of negligence with regard to any and all aspects of participation in the as above mentioned work.

GOVERNING LANGUAGE

This Agreement is drafted in English. In the event of any ambiguity, conflict, or inconsistency between any translation (including the Chinese version) and the English version regarding interpretation or meaning, the English version shall ultimately prevail. 

ACKNOWLEDGEMENT AND SIGNATURES

I have read, understood and agree to the above terms and conditions including the RELEASE AND WAIVER, and confirm that I am signing this document freely, having been fully informed as to its contents and implications.
