Would You Like a Design on the Back?
Examples below are included in the cost. If you would like something different, there is an additional cost depending on the complexity of the design.

Pageant System (if you would like it included)
If you want your pageant's logo included, you must supply it as a high quality png or jpg. I will not retrieve it myself and will not be responsible for the quality of the logo.

Terms of Service

Final design will be delivered electronically to the email address above within 7-14 days of the receipt of this form, unless otherwise notified. There will be 1 (one) opportunity to request reasonable edits to the design. All payments are non-refundable. By clicking the "Submit" button below, you hereby give Designs by BMG permission to use the attached photos to create your chosen design. Please be sure that you have permission from the owner of the photos to do so, as it is not the responsibility of Designs by BMG to obtain a photo release. You also hereby state that all information above is correctly inputted, spelling and size included. It is the buyer's responsibility to have the design printed, and Designs by BMG will not be held responsible for misprints.
