Payment Policy: Payment for all fees incurred are due in full at the time services are rendered. Estimates/Treatment Plans are available upon client request. Appointments that are "No Call/No Show" may be subject to a fee in the amount of the exam prior to rescheduling. New clients with exotic pets are required to pay a deposit in the amount of the exam fee upon booking their appointment. The deposit is nonrefundable if the appointment is a "No Call/No Show".
Cancelation policy: Appointments must be canceled at least 24 hours prior to the scheduled appointment time. Cancelations that occur with less than a 24 hour notice will result in a charge in the amount of the exam. We understand that extenuating circumstances do arise. Please discuss with a member of our Customer Service Representative team for more information.
Veterinarian-Client-Patient-Relationship (VCPR): The VCPR forms a foundation of understanding between the veterinarian and yourself regarding the care of your pet(s A VCPR requires a veterinarian to assume the responsibility for providing care for the patient, and the client has agreed to follow the veterinarian's instructions. The veterinarian must have recently physically examined the patient and be personally acquainted with the keeping and care of the patient in order to provide medical treatment(s) and/or dispense prescription medications.
Client Authorization: I hereby wish to establish a VCPR with the attending veterinarian(s) of Hillside Pet Clinic and agree to the above terms of service. By signing this form, I affirm that I am the legal owner (or agent acting on behalf of the owner) and agree to assume all legal and financial responsibilityfor the patient being presented for veterinary care.
