Mentee Application

Informed Consent and Acknowledgement

I, the undersigned, hereby state that if my child is accepted as a mentee, I agree to abide by the rules and regulations of the FRNDS Inc. I understand that the program involves maintaining consistent meetings during academic years at the assigned location or virtually. Further, I understand that attendance at programs and meetings is expected (unless absence is approved) and understand that repetitive absences on my child’s part may result in suspension and/or termination of the mentoring relationship. I hereby acknowledge that my child may be transported by his/her mentor while participating in the mentorship program and that such transportation is voluntary and at his/her own risk. I authorize the release of information from my child’s school to FRNDS Inc - including but not limited to: Report Cards, Attendance records, Immunization Records, IEPs and 504 plans.  

Further, I hereby fully release, discharge, and hold harmless FRNDS Inc, participating organizations, and all of the foregoing officers, directors, coordinators and volunteers both collectively and individually from any and all liability, claims, causes of action, costs and expenses that may be or may at any time hereafter become attributable to my child's participation in the FRNDS Inc program.

Image and Video Release and Authorization

I, authorize FRNDS Inc, and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me, or members of my family, for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium. I hereby release FRNDS Inc. and its legal representatives from all claims and liability relating to said images or video. Furthermore, I grant permission to use my statements that were given during an interview or guest lecture, with or without my name, for the purpose of advertising and publicity without restriction. I waive my right to any compensation.

Virtual Participation Consent and Acknowledgement

I grant permission for my child/children to participate in virtual programming and mentor meetings via virtual platforms (Zoom, FaceTime, Microsoft Teams or Google Video). Meeting links will be sent directly to the mentee's email. It is recommended an email be created for the mentee if they do not have one already.  Students must refrain from posting any derogatory statements within chat features in FRNDS programming. Students are not allowed to forward member only virtual program links to students who are not enrolled in the program. Failure to comply will result in termination from the program.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
