I here by consent to receiving a sunless spray tan by House Of Mist.

Exfoliating and shaving should be done 24 hours before your tan. Wearing deodorant, lotions and makeup will act as a barrier causing an uneven tan. So we ask that you not wear these before your tan.

Please refrain from activities that may make you sweat during the development time.

It is recommended wear loose dark fitted clothes for after the appointment to avoid any rubbing of the skin.

After you rinse from the recommended time. You should moisturize two times daily, avoid hot baths, hot tub's.

To my knowledge I have no medical conditions or allergies that would perclude me from having this sunless tan.

I agree to hold House Of Mist harmless of any and all medical complications that may arise. Consult a physician if a reaction occurs.
