I would like to play on the same team with following players. League organizers will form teams, in part, based on this buddy list and your skill level.

Important Notes

· Event Dates and Times: 7/10/23 - 7/12/23 from 9:00am - 2:00pm

· Check-in begins 7/10/23 at 8:00am

· Please have all equipment ready - including mouth guards, helmet, cleats, stick, elbow pads, gloves, and chest protection equipment that meets NOCSAE performance standard ND200.

· Failure to have proper equipment will result in non-participation status (no refunds as a result of lack of preparation).

· Pinneys will be provided for event.

· Food will NOT be provided for the event.

· No food on the field.

· It is expected each player drink a gallon of water each day to remain hydrated

· Clean up after yourself and others if necessary.

· The league organizers reserve the right to form the teams to ensure the talents of all teams are balanced.

· Fee: $140 per participant. Mail in checks to Forest Lacrosse LLC. 131 Hampton Rd. Syracuse, NY 13203. ---- Will accept checks or cash day of event. Must pay before participation is allowed.

· NO REFUNDS WILL BE GIVEN IN THE EVENT OF INABILITY TO ATTEND
