Registration Fees 

Please submit registration fee (cash or check) at the Parish office.  Please provide student name and program's name when submitting payment. (i.e. John Smith, RE-English). 

You may also pay through Venmo @SantiagoDeCompostela (QR code below).

Fees include all materials.

 

 

• First Year: $120 (due at the time of registration)
  
• Second Year: $120 (due at the time of registration)
  
• First Communion Retreat:$40 (due by december 2023)  Please pay the retreat fee separately 
• After First Communion (this is a class for children who have already received the First Communion) : $65 (due at the time of registration)

 

Registration Fee and child’s Baptism Certificate must accompany this form.

If your child has not been baptized please email the birth certificate to: aalvarez@sdccatholic.org

From time to time, we take photographs, videos, recordings, of the Religious Education Program students at classes, events and gatherings. We would like to be able to use these photographs, videos, recordings, for flyers, parish and diocesan publications, and the parish website and social media. To do this, we need the parents’ consent. We will not use the last names of any individual whose photos, videos, or recordings are posted. If there are concerns about the photographs, videos, recordings,posted on the website or parish's social media, please contact the R.E. Coordinator or webmaster and they will promptly be removed.

I(We) the undersigned, parent(s)/Guardian(s) of the student named below, authorize and give full consent, without limitation or reservation, to Santiago de Compostela, to publish any photographs, videos, recordings, in which the below named student appears while participating in any program with the Santiago de Compostela Religious Education Ministry, or with the Diocese of Orange events. There will be no compensation for use of any photographs, at the time of publication or in the future. I (we) also agree to the dissemination and publication of any written work authorized and submitted by our student in any Diocesan, and/or Santiago de Compostela Catholic Church sponsored contests or events. I (we) hereby waive any rights to compensation for any media contemplated herein and any rights that I (we) otherwise might have to limit or control such making or use of any media contemplated herein.

LIABILITY WAIVER

Hereby give my permission for my child participation in planned activities. I agree to direct my child to cooperate and follow directions and instructions of parish, or diocesan personnel responsible for all activities.As a condition of my child being allowed to do so, I hereby release and discharge Santiago de Compostela Catholic Church, the Diocese of Orange, its constituent organizations, including but not limited to The Roman Catholic Bishop of Orange, a Corporation sole, and their officers, employees and volunteers from any and all claims for personal injuries or damages which are caused by the negligence, active or passive, of any of the entities, individuals, names described above.

I agree, that in the event of injury as a result of participation in planned activities, including transportation to and from these activities, whether or not caused by the negligence, active or passive, of the parish, school, or diocesan youth activities program or any of its agents, employees or volunteers, recourse for the payment of any resulting hospital, medical, dental treatment or related costs and expenses will first be had against any accident, hospital, medical or dental insurance, or any available benefit plan of mine or my spouse. I am not aware of any medical condition which would render it inappropriate for participation in any activity.

I hereby give permission to the physician, nurse, dentist or licensed care staff selected by the supervisory personnel then present to render medical, dental or other appropriate treatment deemed necessary and appropriate by the physician, nurse, dentist or licensed care staff.

We rely on volunteers to offer the youth of our parish the valuable programs we have designed. Please consider working with us to help our children grow in faith. Volunteers who commit to helping throughout the year will receive a discount on the registration fee**.

Note: Completion of the live or online safe environment training course is required prior to commencement of service.

I certify that this volunteer application was completed by me and that all of the information on this application is true and correct to the best of my knowledge. I understand that any falsification, misrepresentation, or omission of facts called for herein will result in my disqualification from further consideration as a volunteer. I understand that this volunteer application is not valid without my signature.

PARENT PLEDGE

In accordance with the Bishop of the Diocese of Orange, I have chosen to enroll and participate in my child’s Faith Formation at Santiago de Compostela Church. I understand that the parish provides a place for my family to grow and be nourished in our personal relationship with Jesus through prayer, the study of scripture, generosity, and liturgy. I promise to support my child’s faith formation at church and home by:

• Practicing my faith and being an example to my family.
• Attending and participating in Sunday Mass every week.
• Daily prayer such as morning offering, night prayer, grace before and after meals,
• Attend all the Conferences for Parents planned during the school year.                                    
• PUNCTUALITY: Being on time is an excellent way to show your child that attendance in Religious Education is important.

More than two (2) unexcused absences will be cause for evaluation of the student’s placement in the program. Sports or extracurricular activities do not fall into the category of excused absences.              

COMUNICATIONS

I authorize Santiago de Compostela and the Faith Formation Program to send me by e-mail, text messages, and REMIND messages,  all the information about the catechesis and the different celebrations in the Parish, such as Masses, classes, parents meetings, training sessions for parents and guardians, summer camp, among others.
